Weight Management
Initial visit $ 225.00 which includes your first 2 visits, EKG, complete medical history and physical, body composition and metabolic rate testing. This visit typically lasts an hour and includes thorough review of your diet and exercise habits. Dr. Heim will create a program based on your individual needs which may include meal replacement supplements, dietary and exercise instruction, reccomendation for nutritional supplements, review of blood work, and prescription medication if needed.
Follow-up visits: $50.00 Follow up visits include vitals, complete body composition and educational sessions with Dr. Heim who will teach you proper nutrition and exercise to accomplish life long weight loss.
Initial Visit including basic bloodwork: $270.00 labwork includes:(cholesterol panel, metabolic profile, complete blood count and thyroid panel).
Meal Replacement Supplements: full program $45.00/week
Anti-Aging/Bioidentical Hormone Replacement Therapy
Initial Visit: $225.00 This visit will last 60 minutes and includes full body composition and metabolic rate testing, medical history and physical and a thorough review of your current lifestyle habits. You will be intructed on lifestyle changes aimed at maximizing health and slowing down the aging process including proper diet, exercise, stress reduction and recommendations on nutritional supplements based on your unique needs. Hormone replacement therapy will be prescibed if needed based on blood work and symptoms.
Follow-up visits: $125.00 (every 3 – 6 months) these visits will last 30 minutes and will include review of follow up bloodwork, full body composition testing, and any alteration of hormone replacement therapy as needed.
Osteopathic Manipulation
Initial Visit: $125.00 Follow-up visits: $50.00
Primary Care
New Patient office visits start at $150.00
Established Patient visits start at $75.00.
Phone/email consultation: $50.00 – once you are an established primary care patient, Dr. Heim may prescribe medications after a brief phone or email consultation for simple conditions such as mild upper respiratory or urinary tract infections.

We are primarily a fee for service organization which means that payment is due at time our services are performed. Patients are then given a copy of their bills and forms to submit their claims to their insurance companies for reimbursement. Many Insurance companies and PPOs reimburse up to 80% of your bill for office visits. Typically insurance companies do not reimburse for weight management. Check with your insurance carrier for information about what percentage they cover for out of network providers. We do accept Health Savings Accounts and Flex spending accounts as well as Cash, Debit, Visa, Discover and Master Card. Patients may still use their insurance for any labwork, diagnostic testing, and specialist visits. We do have special pricing available through a local lab for bloodwork if you are uninsured or have a high deductible insurance plan. It may cost less to pay cash for blood work under our special contracts than to submit to your insurance company.
